For attendance in all grades, children need the following:

  • 4 doses of tetanus (1 dose on or after the 4th birthday)
  • 4 doses of diphtheria (1 dose on or after the 4th birthday)
  • 3 doses of polio
  • 2 doses of measles
  • 2 doses of mumps
  • 1 dose of rubella (German measles)
  • 3 doses of hepatitis B
  • 2 Doses of varicella (chickenpox) vaccine or history of disease
Emergency Cards

Emergency cards are distributed to all students on the first day of school. It is very important that this card is completed and promptly returned to school. Complete information is necessary to ensure your child’s health and safety while at school. All information is confidential. Please provide insurance information to verify student health care coverage. If a child becomes ill or is injured at school, I will attempt to notify the parent/guardian. However, if this is not possible, emergency contacts listed on the reverse side of the emergency card will be notified and the student will be released to them.

Please make sure your emergency contacts are local and known to your child. Please also inform your emergency contacts that they need to provide proper identification, such as a driver’s license, so that your child may be safely released to their care. In the case of a life-threatening illness or injury, the student will be transported by EMS (emergency medical services) to the nearest medical facility. Please update any necessary information during the school year, including change in phone numbers and change in employment, so that we are able to contact you in the event of an emergency.
