The Columbia Borough School District performs an annual census of the District residents in February. The District mails census forms directly to residences for completion, rather than sending census takers door-to-door in an effort to save money and to make the census more convenient for residents to complete.
The District is required by the PA School Code to collect a careful, correct and accurate census of all households (with or without children) in our district. Census data provides information necessary for the District to fulfill the obligations with regard to compulsory attendance regulations, effectively plan for special education services and for the calculation of future enrollment trends. All information provided is confidential.
When you receive your pre-printed form, please make any necessary corrections, sign and date the form and return it in the enclosed envelope or bring it in – whether there are corrections or not – to the Tax Office at 200 N. 5th Street. If you would rather fax the form to our office, please fax your form to the attention of the Tax Office – 717-681-2617. If you lose your form, you can print a blank on the link below. (You will need Acrobat Reader to view this form.) The form will only take a few minutes of your time. In instances where the census forms are not returned, the District will have to incur additional costs to contact by phone or in person by a census taker. Your cooperation in returning your form will be appreciated.
